Mission Statement

Our mission at Jeffrey E. Goldberg, MD is to provide quality health services and facilities for our community, to promote wellness, to listen carefully to the concerns  of our patients and parents, answer all questions thoroughly, and carefully explain our findings and  recommendations to our patients. We pride ourselves in providing expert care in a  comfortable and unrushed environment.

Patient Forms

For your convenience, all new patient forms can be completed prior to your appointment by using our Patient Portal. Dr. Goldberg recently affiliated with ENT of Georgia North. This affiliation allows us to offer our patients the benefit of secure portal access, electronic medical records, and other technology services.

Patient Portal

Electronic Health Records, or EHRs, are a computer based system of patient medical records that focus on the total health of you, the patient. Everything that had been written by hand is now entered into a computer. In order to improve our patients’ experience, Dr. Goldberg has joined wtih ENT of Georgia. By affiliating with a large and robust practice, Dr. Goldberg’s patients can benefit from a well established electronic heath record system. Your records are updated, and immediately accessible to your provider at any office of ENT of Georgia. You also have access to our secure Patient Portal.

Our practice accepts most major insurance plans and most of the smaller insurance plans. If we do not participate with your insurance and your plan allows out of network benefits, our office staff will assist you with the necessary paperwork so you can receive reimbursement directly from your insurance company.

If you do not see your insurance listed below, please call the office at 770-495-3820 to speak with one of our knowledgeable staff members about your individual coverage.

Our Providers reserve specific time on their schedules for each patient.We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. Please extend this courtesy so that other patients may be seen.

Patients who do not keep their appointment, or cancel with short notice, will be subject to a $25.00 fee.

We provide 24-hour emergency coverage for all patients. If you have an emergency after office hours, call our office at 770-495-3820. The answering service will take your message and contact Dr. Goldberg or a covering physician immediately.
